Process

Representatives of health regions are being asked to determine the optional content to be asked of respondents in their region by choosing from a predetermined list of questionnaire modules.

All modules on this list have been assigned a point value based on the average length of time needed to respond to the module. The total point value of all modules selected by a health region must not exceed 32.

Health region selections will be communicated to Statistics Canada by designated provincial / territorial representatives.

Deadline:

All selections must be received by Statistics Canada no later than Friday, October 29, 2004. Some regions have requested that they be able to modify their selections based on the decisions of others within their province or peer group. To permit such modifications, all health regions are asked to record preliminary selections no later than Friday, September 24.
