1. In Newfoundland and Labrador, six community health regions were reduced to four Regional Integrated Health Authroities (RIHAs). Boundaries are unchanged for two RIHAs and the other four have been collapsed into two. Name and code changes in effect as of March 1st, 2005.
2. The six zones are aggregations of the nine District Health Authorities (DHA). Zones 1, 3 and 4 are each comprised of two DHAs. The remaining three zones change in name only with the following small exception. Mount Uniacke area, previously part of Zone 3 is cut-off by new DHA 4 boundary. Statistics for this area (population 1,114) will be included with DHA 9 (Halifax area). As a result, there is high comparability between Zone 6 and DHA 9 and between Zone 3 and DHA 4/5.
3. District health councils (DHC) were dissolved. 14 Local Health Integration Networks (LHINs) came into effect April 1st, 2005.
4. As of April 1, 2005 Muskoka-Parry Sound Health Unit dissolved. The Muskoka part of this region was combined with Simcoe County District increasing the population by 14%, and the Parry Sound part joined North Bay and District, increasing the population by 30%.
5. For most data sources (with the exception of Census and Demographic population estimates), health region level data are not available for some northern health regions in Manitoba and Saskatchewan which have small populations. To avoid suppression in these areas where small numbers or sample size impact on data quality, data have been grouped with neighbouring regions, as follows: Churchill Regional Health Authority, Manitoba (4690) is combined with Burntwood Regional Health Authority (4680) and referred to as "Burntwood/Churchill (4685)"; and, Athabasca Health Authority (4713) Saskatchewan is combined with Mamawetan Churchill River Regional Health Authority (4711) and Keewatin Yatthé Regional Health Authority (4712) and referred to as "Athabasca/Keewatin/Mamawetan (4714)"
6. In December 2003 Calgary Health Region gained a population of approximately 13,000 (or 1.2%) from David Thompson Regional Health Authority which lost 4.4% total population.
